A small update to Form Submission Alert Emails

What's Changing:

The form submission email alert has been updated slightly:

• The "Additional visitor information" section which contains the IP address and GeoIP look up fields will now display only if the "Also include visitor's GeoIP information" checkbox is checked on the Form Alert setup. 
• In response to customer feedback, the header of the form alert has been reduced in size, making it easier to get to the form submission data with less scrolling. 

Customer Impact:

Any customers who receive the form alert will notice these changes.  You may need to adjust the Form Alert set up to include the visitor Geo IP information if you notice it is no longer appearing on your forms.
