Updates to how Survey results are displayed

We've streamlined how Survey results appear in the Sent Message Report.

Previously, responses on email surveys and the associated comments were displayed as Clickthrough's in the Sent Message Report.

The report now shows only link clicks in the Clickthrough's graph and all comments have been moved to Survey Participants alongside the participant's name and email address. Only the most recent comment is shown.
