Adding File Upload to a Form

The File Upload block allows Form submitters to upload documents with their Form submissions.

For example, this could include resumes, article submissions, or statements of work. Files can be retrieved by accessing the Act-On list that collects the submissions.

This functionality is only available for Audience Center Forms - those that submit to All Contacts.

Instructions

Use File Upload Field

  1. Go to Content > Forms
  2. Edit an existing Form, or create a new Form.
  3. In the Design tab, drag the File Upload block to your Form:

  1. Select the File Upload block to configure it in the right-hand sidebar:
    • Label tab:
      • Customize the Input Label for your file upload block.
      • Configure the file upload text & Browse Button.
      • Customize the Form Field Name for your files.
    • Design:
      • Toggle on to Allow Multiple Files Uploads (up to 10).
      • Customize the Browse Button Style.
      • Customize the file Upload File Zone Style.
    • Validation:
      • Choose to make the file upload required.
      • Choose if you want to restrict the file type. You can select multiple types from the list or type in your own file type extensions, for example:

      • Customize the Error Messages for your validations and the general error messages.
  1. At the bottom of the sidebar, click OK.

You can add multiple different file upload blocks if you want to designate specific file types for each and track the uploads in separate fields.

Access Files Submitted Via File Upload

From the Form Submission List

  1. Go to Audience Center > All Contacts & click Form Submissions.
  2. Click on the Form Submission list that has your file uploads
  3. For an individual submission, hover over the row to see the menu on the right. From here you can preview the files or download them. You can also click on the file field to download them:

  1. Use the menu at the top of the list to download the entire list. You'll have the option to download with attachments, which will include a .csv file of the submission data plus a folder of all attachments in a .zip file.
    • The attachments will include the Form submission list ID, the contact name, and the file name in their name.

From the Form Report

  1. Go to Content > Forms, hover over the Form that has your file uploads, & click View report to see the report for the Form.
  2. On the Report tab (top left), select Form Responses to see the total # of submissions with a file, and the total files uploaded.
  3. From the Actions menu (top right), click Download. You'll have the option to download with attachments, which will include a .csv file of the submission data plus a folder of all attachments in a .zip file.
    • The attachments will include the Form submission list ID, the contact name, and the file name in their name.
If you are posting to the Form from an external Form source, the file upload field will contain the URL of the external Form's file upload. However, you will need to visit that Form's source to download the files.
